Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Santa Rosa, CA. Key Responsibilities:
• Provide physical therapy services to home health patients
• Visit up to 6 patients per day while maintaining 30 productivity points standard
• Maintain current BCLS certification
• Complete required modules and training programs pre-start and annually
• Work 1 weekend shift per contract
• Utilize Epic and Oasis charting systems for documentation
• Provide reliable transportation for patient visits within service area
• Complete Oasis assessments and documentation
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete required compliance documentation and certifications
• Self-report time spent on non-billable training modules
• Maintain valid state physical therapy licensure
• Adhere to facility policies and procedures for patient care delivery
• Work 8-hour day shifts as scheduled
• Submit monthly cell phone expenses if facility-provided communication device not available
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
