Position Summary:

VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Days at a leading SNF in Elkins, WV. Key Responsibilities:

• Provide nursing care services as a Licensed Practical Nurse in a skilled nursing facility setting
• Work 12-hour shifts from 07:00 to 19:00
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and document changes in condition
• Assist with activities of daily living and patient mobility
• Maintain accurate patient records and documentation
• Collaborate with healthcare team members to ensure quality patient care
• Follow infection control protocols and safety procedures
• Communicate effectively with patients, families, and healthcare staff
• Maintain current LPN licensure and required certifications
• Adhere to facility policies and procedures
• Comply with dress code requirements including designated scrub colors

Job Requirements:

Active, unencumbered license in WV
Minimum of 1-7 years of relevant experience, depending on specialty
Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

Current Resume listing detailed work history (MM/YYYY format)
Two current clinical references
State-required professional license
Physicial exam (completed within the past 12 months)
TB skin test or Quantiferon Gold (within the past 12 months)
Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
Current Tdap and Flu vaccinations
Must pass background check and drug screening
