Requirements:

Active license in NJ
Minimum of 1 year of recent experience in LPN
Current BLS (and other certs as required: ACLS, PALS, etc.)
Up-to-date resume with recent work history
2 professional clinical references
TB test and physical within 12 months
Immunizations/declinations or titers as needed
Respirator fit test within 12 months
Must pass background check and drug screening
