Requirements:

Active license in MD
Minimum of 2 years of recent experience in SNF
Current BLS (and other certs as required: ACLS, PALS, etc.)
Up-to-date resume with recent work history
2 professional clinical references
TB test and physical within 12 months
Immunizations or titers for MMR, Varicella, Hep B, Tdap, and flu
Respirator fit test within 12 months
Must pass background check and drug screening
