Position Summary:
VitalHire is seeking a skilled and compassionate Social Services - MSW (Masters of Social Work) to join our growing clinician community. Key Responsibilities:
• Support the clerical needs of the Case Management Department including insurance authorizations, post-acute care referrals, and delivering Medicaid notices to patients
• Assist with pre-authorization reviews and benefits eligibility verification
• Coordinate services to home care and equipment following discharge
• Arrange transfers to physician appointments and facilitate discharge home
• Refer patients to rehabilitation and long-term care facilities
• Refer patients to other disciplines and resources as needed
• Document pertinent information to facilitate care coordination
• Coordinate with internal and external team members
• Educate patients and caregivers to facilitate discharge planning
• Conduct needs assessments and order durable medical equipment
• Manage average caseload of 18-24 patients (up to 30 patients on weekends)
• Utilize Epic charting system for documentation and case management
• Work with adult and geriatric patient populations
• Collaborate with interdisciplinary support teams including Physical Therapy, Interpretation Services, Social Services, Pharmacy, and Transportation
• Complete required orientation and training modules pre-start and annually
• Float between hospital units and sister facilities as needed
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Maintain schedule flexibility between 0730-1600 or 0800-1630 Monday through Friday
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
