Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Middletown, RI. Key Responsibilities:
• Provide direct patient care in skilled nursing facility setting
• Assist residents with activities of daily living including bathing, dressing, and mobility
• Monitor and document vital signs and patient condition changes
• Maintain accurate documentation and charting of patient care activities
• Follow care plans and physician orders for assigned residents
• Assist nursing staff with medication administration and treatments
• Ensure resident safety and comfort throughout shift
• Communicate effectively with residents, families, and healthcare team members
• Maintain clean and organized resident living spaces
• Respond promptly to resident call lights and requests for assistance
• Work collaboratively with interdisciplinary team members
• Adhere to infection control protocols and safety procedures
• Participate in resident care conferences and planning meetings as needed
Job Requirements:
- Active, unencumbered license in RI
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
