Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Plainview, TX. Key Responsibilities:
• Provide physical therapy services for adolescents, adults, and geriatric patients • Maintain current BLS certification and valid state licensure • Complete required modules and training programs pre-start and annually (approximately 16 hours) • Work assigned schedule: Monday-Thursday 7:30-18:00 and Friday 8:00-12:00 • Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Utilize Epic charting system for documentation and patient care • Conduct patient assessments and evaluations • Develop and implement treatment plans and home exercise programs • Provide gait training and mobility assistance including walker and wheelchair assessments • Educate patients and families on treatment plans and injury prevention • Perform orthopedic rehabilitation including post-surgical care for hip and knee replacements • Document patient progress, reassessments, and discharge planning • Maintain 1:1 patient ratio as required • Float between sister facilities as needed • Provide minimum of 2 supervisory references covering 1 year of experience within last 3 years • Complete required compliance documentation and background checks • Self-report time spent on non-billable training modules • Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in TX
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
