Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Assess patients and coordinate care for adults and geriatrics with patient ratios of 1:25 • Perform discharge planning and coordinate post-acute care services • Organize and participate in family meetings • Participate in the development of treatment plans • Coordinate with HMOs, IPAs, and other insurance providers • Arrange durable medical equipment (DME), skilled nursing facility placements, and ambulance transport independently • Handle cases involving adoption/foster care, substance abuse treatment, homelessness, mental health crises, and protective services • Process advance directives and POLST forms • Manage appeals and denials processes • Navigate CMS guidelines and DRG requirements • Ensure compliance with HIPAA guidelines and OSHA requirements • Utilize Epic charting system for computerized documentation • Maintain knowledge of The Joint Commission standards and National Safety Goals • Float between sister facilities as required • Work weekend rotation shifts to meet unit needs • Complete 40 hours of orientation and required pre-start modules • Complete annual training modules and maintain compliance documentation • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Maintain valid state licensure and travel nursing experience • Work 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Self-report time spent on non-billable training modules • Utilize facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
