Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Assess patients and coordinate care for case loads up to 25 patients on days and 45 patients on weekends • Perform discharge planning and utilization review services • Organize and participate in family meetings as part of interdisciplinary care team • Participate in the development of patient treatment plans • Work with HMOs, IPAs, and other insurance providers for authorization and coverage decisions • Provide case management services for diverse patient populations including neonates, adolescents, adults, and geriatrics • Coordinate services for patients with complex social needs including adoption/foster care, drug/alcohol abuse treatment, homelessness, and mental health crises • Manage advance directive and POLST documentation processes • Coordinate with protective services when required • Arrange durable medical equipment (DME), skilled nursing facility placements, and ambulance transport independently • Collaborate with interdisciplinary support team including Physical Therapy and Social Services • Utilize Epic charting system for computerized documentation • Work minimum of 4 weekend shifts in a 6-week period • Complete 40 hours of orientation upon assignment start • Float between facilities within the state as required • Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Complete required pre-start and annual training modules • Maintain current licensure and required certifications • Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty setting • Use facility-provided communication devices for work-related duties when required
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
