Position Summary:

Key Responsibilities:
• Provide nursing care services as a Licensed Practical Nurse in skilled nursing facility
• Work Monday, Friday and every other weekend schedule
• Maintain current state LPN licensure
• Deliver patient care from 06:45 to 19:15 shift hours
• Administer medications per physician orders and facility protocols
• Monitor and document patient vital signs and health status
• Assist with activities of daily living and patient mobility
• Collaborate with interdisciplinary healthcare team members
• Maintain accurate patient documentation and medical records
• Follow infection control and safety protocols
• Communicate patient changes and concerns to supervising registered nurses
• Adhere to facility policies and procedures for nursing care delivery
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
