Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Evenings at a leading SNF in Jersey Shore, PA. Key Responsibilities:
• Provide nursing care services as Licensed Practical Nurse in skilled nursing facility setting
• Work bi-weekly rotating schedule including every other weekend
• Make up hours the following weekend if calling off on Friday, Saturday, or Sunday per facility policy
• Work evening shift from 3:00 PM to 11:30 PM
• Maintain current LPN licensure and certifications
• Administer medications and treatments as prescribed
• Monitor patient vital signs and document changes in condition
• Assist with activities of daily living and patient mobility
• Collaborate with healthcare team members on patient care plans
• Maintain accurate and timely documentation of patient care
• Follow facility policies and procedures for patient safety
• Respond to patient emergencies and call situations
• Assist with admission, transfer, and discharge processes
Job Requirements:
- Active, unencumbered license in PA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
