Position Summary:

Key Responsibilities:
• Provide nursing care services as an LPN in a skilled nursing facility setting
• Work scheduled shifts on Saturday, Sunday, and Monday from 11p-7a
• Maintain availability for additional hours on 11-7 or 3-11 shifts as needed
• Deliver patient care according to facility policies and nursing standards
• Maintain current LPN licensure and required certifications
• Document patient care activities and assessments accurately
• Administer medications as prescribed and within scope of practice
• Monitor patient conditions and report changes to supervising RN or physician
• Assist with activities of daily living for residents as needed
• Collaborate with interdisciplinary healthcare team members
• Follow infection control protocols and safety procedures
• Participate in required training and continuing education programs
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
