Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Napa, CA. Key Responsibilities:
• Provide physical therapy services in home health setting with 1:1 patient ratios
• Conduct up to 6 patient visits per day meeting 30 productivity points standard
• Maintain current BCLS certification
• Work 1 weekend shift per contract period
• Utilize EPIC charting system for patient documentation
• Provide own reliable transportation for patient visits with occasional travel up to 50+ mile radius
• Complete required pre-start and annual training modules (average 16 hours, non-billable)
• Self-report time spent on training modules with completion recorded in Workramp transcripts
• Maintain valid state physical therapy licensure
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Float between all facilities within the state as required
• Work 8-hour day shifts as scheduled
• Treat patients with various conditions including post-operative care, neurological disorders, and orthopedic conditions
• Develop and implement home exercise programs for patients
• Provide patient and family education on treatment plans and care
• Submit monthly cell phone bills if facility-provided communication devices are not available
• Comply with all facility policies and procedures for patient care delivery
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
