Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Wayne, PA. Key Responsibilities:
• Provide direct patient care services as a Certified Nursing Assistant
• Assist patients with activities of daily living including bathing, grooming, and mobility
• Monitor and document patient vital signs and conditions
• Maintain accurate patient records and documentation
• Assist nursing staff with patient care procedures
• Ensure patient safety and comfort at all times
• Follow infection control protocols and safety procedures
• Communicate effectively with patients, families, and healthcare team members
• Maintain current CNA certification and complete required continuing education
• Adhere to facility policies, procedures, and standards of care
• Assist with patient transfers and positioning
• Support patients with nutritional needs and feeding assistance
• Maintain clean and organized patient care areas
• Report changes in patient condition to supervising nurses
• Participate in care planning and team meetings as required
Job Requirements:
- Active, unencumbered license in PA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
