Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Evenings at a leading LTC in Seaford, DE. Key Responsibilities:
• Provide licensed practical nursing care in accordance with state regulations and facility policies
• Work 2 week pattern schedule with every other weekend requirement
• Maintain current LPN licensure and certification
• Administer medications and treatments as prescribed
• Monitor patient vital signs and document observations
• Assist with patient care activities and daily living needs
• Collaborate with healthcare team members to ensure quality patient care
• Maintain accurate and timely documentation of patient care
• Follow infection control protocols and safety procedures
• Communicate patient status changes to appropriate healthcare personnel
• Participate in continuing education and professional development activities
• Adhere to HIPAA regulations and maintain patient confidentiality
Job Requirements:
- Active, unencumbered license in DE
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
