Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Cottonwood, AZ. Key Responsibilities:
• Provide physical therapy services in acute care setting
• Manage patients with moderate to maximum assistance needs
• Conduct patient evaluations and treatments
• Work with ventilated patients
• Perform co-treatments with other healthcare professionals
• Adapt to changing patient needs and pick up additional evaluations throughout the day
• Work with patients presenting various medical diagnoses
• Maintain current Arizona state physical therapy license
• Maintain current BLS certification through American Heart Association
• Obtain and maintain Arizona Fingerprint Card
• Work Tuesday through Saturday schedule (5x8 hours, 0800-1630)
• Work weekend days as required
• Float to surrounding clinics as needed
• Utilize Cerner charting system for documentation
• Complete required modules and training during orientation
• Adhere to facility quality and patient safety standards
• Follow medication administration protocols (BCMA)
• Maintain business casual dress code
• Provide two professional references covering required timeframes
• Complete compliance documentation within specified deadlines
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
