Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Nights at a leading LTC in Clayton, NM. Key Responsibilities:
• Provide nursing care as a Licensed Practical Nurse in a 45 bed rural facility
• Work as the sole nurse per shift providing comprehensive patient care
• Maintain current LPN licensure and certifications
• Administer medications and treatments as prescribed
• Monitor patient vital signs and document changes in condition
• Collaborate with physicians and other healthcare team members
• Maintain accurate patient documentation and medical records
• Respond to patient emergencies and provide appropriate interventions
• Educate patients and families on healthcare procedures and discharge instructions
• Ensure compliance with facility policies and regulatory standards
• Manage multiple patients independently during assigned shifts
• Communicate effectively with incoming shift staff during patient handoffs
Job Requirements:
- Active, unencumbered license in NM
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
