Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Mount Angel, OR. Key Responsibilities:
• Provide physical therapy services to home health patients in assigned geographic areas
• Conduct patient assessments and develop individualized treatment plans
• Perform OASIS assessments and documentation as required
• Visit up to 6 patients per day maintaining 30 productivity points standard
• Provide one-on-one patient care during home visits
• Utilize Epic charting system for all patient documentation
• Complete required pre-start and annual training modules
• Maintain current BCLS certification
• Work 5 days per week on 8-hour day shifts
• Provide reliable transportation for patient visits within 50+ mile radius
• Work occasional weekends as needed
• Maintain current state physical therapy licensure
• Complete required COVID-19 vaccination series plus booster
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Self-report time spent on non-billable training modules
• Submit work-related cell phone expenses if facility communication device not provided
• Complete required compliance documentation and certifications
• Adhere to facility policies and procedures for home health service delivery
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
