Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Middletown, RI. Key Responsibilities:
• Provide licensed practical nursing care to patients in accordance with state regulations and facility policies
• Administer medications and treatments as ordered by physicians
• Monitor patient vital signs and document changes in patient condition
• Assist with patient assessments and care planning
• Maintain accurate and timely documentation of patient care activities
• Collaborate with healthcare team members to ensure optimal patient outcomes
• Follow infection control protocols and safety procedures
• Provide patient and family education as appropriate
• Respond to patient emergencies and call for assistance when needed
• Maintain current LPN licensure and required certifications
• Work assigned shifts including Tuesday, Thursday, and Saturday
• Adhere to professional standards and ethical guidelines for nursing practice
Job Requirements:
- Active, unencumbered license in RI
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
