Position Summary:
VitalHire is seeking a skilled and compassionate LPN - SNF to join our growing clinician community. This high-impact opportunity offers a 3x12 Nights at a leading LTC in Hadley, MA. Key Responsibilities:
• Provide licensed practical nursing care services
• Maintain current state LPN licensure
• Deliver patient care in accordance with nursing protocols and procedures
• Document patient care activities and maintain accurate medical records
• Administer medications as prescribed and within scope of practice
• Monitor patient vital signs and report changes to supervising registered nurse
• Assist with patient assessments and care planning
• Maintain professional competency through continuing education requirements
• Collaborate with healthcare team members to ensure quality patient outcomes
• Follow infection control protocols and safety procedures
• Provide patient and family education as appropriate
• Maintain patient confidentiality and comply with HIPAA regulations
Job Requirements:
- Active, unencumbered license in MA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
