Position Summary:

Key Responsibilities:
• Effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care
• Develop appropriate plan of care following all regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to support clinical interventions
• Administer various procedures as part of the rehabilitation plan including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care
• Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements
• Evaluate treatment outcomes for effectiveness and modify services in relation to patient evaluation
• Make referrals in relation to treatment outcomes
• Provide consultation and counseling to patients, families, caregivers and other service providers related to physical disorders
• Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements
• Document all regulatory and clinically required information in the electronic documentation system
• Maintain patient records according to company policy
• Accurately record treatment minutes for services provided per all regulatory and clinical requirements
• Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings
• Participate in facility and company required trainings, in-services and conferences
• Obtain any necessary DME or supplies per regulatory and clinical practice requirements
• Supervise and support physical therapist assistants, rehabilitation aides and/or students
• Adhere to APTA and State Required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist
Job Requirements:
- Active, unencumbered license in MO
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
