Position Summary:
VitalHire is seeking a skilled and compassionate SLP - LTC to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Leeds, MA. Key Responsibilities:
• Provide speech-language pathology services to patients in accordance with established treatment plans
• Conduct comprehensive speech and language evaluations and assessments
• Develop and implement individualized treatment plans based on patient needs
• Document patient progress and maintain accurate medical records
• Collaborate with interdisciplinary team members including physicians, nurses, and other therapists
• Educate patients and families on treatment goals and home exercise programs
• Participate in team meetings and case conferences
• Maintain current SLP licensure and certifications
• Complete required continuing education units to maintain professional credentials
• Adhere to facility policies, procedures, and safety protocols
• Provide quality patient care in accordance with professional standards and regulations
• Communicate effectively with patients, families, and healthcare team members
• Complete required documentation within established timeframes
Job Requirements:
- Active, unencumbered license in MA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
