Position Summary:

Key Responsibilities:
• Complete Trinity Child and Adult Dependent Abuse Training
• Complete OFAC screening requirements
• Complete Trinity Skills Checklist competency assessment
• Undergo fit testing for respiratory protection
• Complete 9 panel drug screening
• Submit signed Code of Conduct Form
• Complete I-9 employment eligibility verification
• Sign Confidentiality of Patient & Property Information & System Security Agreement
• Complete Integrity and Audit Services documentation
• Complete specialty specific competency testing
• Complete color vision screening
• Maintain current COVID-19 vaccination
• Maintain current MMR vaccination
• Maintain current Hepatitis B vaccination
• Complete tuberculosis screening (2 Step PPD or IGRA)
• Maintain current Varicella vaccination
• Complete required physical/health statement
• Submit Conflict of Interest Disclosure Form
• Complete criminal background check
• Provide education verification
• Complete EPLS screening
• Complete GSA/OIG screening
• Complete Joint Commission testing requirements
• Complete NSO documentation
• Complete System Access Form
• Complete COVID-19 specific health questionnaire
• Maintain valid professional license and certification verification
Job Requirements:
- Active, unencumbered license in IA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
