Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Eureka, CA. Key Responsibilities:
• Provide case management services for patients across multiple age groups including adolescents, adults, pediatrics, and geriatrics
• Manage caseload of up to 25 patients daily
• Coordinate care and discharge planning for patients
• Work with HMOs, IPAs, and insurance providers for prior authorizations and appeals
• Handle admission criteria assessment and benefits eligibility verification
• Process appeals and denials for patient care services
• Conduct needs assessments and order durable medical equipment (DME)
• Manage advance directives, POLST, and protective services processes
• Ensure compliance with HIPAA guidelines and patient confidentiality
• Work with CMS Centers for Medicare and Medicaid Services requirements
• Handle workers compensation case management
• Collaborate with interdisciplinary teams including social services
• Utilize Epic charting system for documentation and case management
• Adhere to Joint Commission core measures and national safety goals
• Float between departments and units based on facility needs
• Work every other weekend as required
• Commit to working 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Complete required pre-start and annual training modules
• Maintain current acute care hospital case management experience
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Complete skills checklist and compliance documentation
• Self-report time spent on non-billable training modules
• Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
