Position Summary:

Key Responsibilities:
• Work rotation schedule of 3 on 2 off 2 on 3 off
• Provide nursing care services as an LPN in skilled nursing facility
• Work night shift from 18:00 to 06:30
• Maintain current LPN licensure and certifications
• Adhere to facility policies and procedures for patient care delivery
• Document patient care activities according to facility standards
• Collaborate with healthcare team members to ensure quality patient outcomes
• Respond to patient needs and emergency situations as required during shift
• Complete required training modules and continuing education requirements
Job Requirements:
- Active, unencumbered license in AL
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
